HCG Levels: 10 Things Fertility Patients Need To Know

If you’re staring at an HCG test result and trying not to panic, you’re not alone. Few moments in early pregnancy after fertility treatment create more anxiety than a line on a home pregnancy test. Or a number on a report.

HCG – short for human chorionic gonadotropin – is the hormone produced after implantation. It’s what home pregnancy tests and HCG blood tests look for. Although HCG can confirm that a pregnancy has started, it doesn’t tell the whole story.

A positive HCG result is a milestone, but only a snapshot. What matters is repeat testing, rising blood numbers, and when the tests are done. And remember: only an ultrasound scan, around four weeks after implantation, can confirm the viability of your pregnancy.   

If you’ve had fertility treatment, HCG scores somehow matter even more. You’ve known every date in your treatment journey, and your test day has been in your diary for weeks. Your emotions, hopes and expectations are sky-high. Understanding the all-important result is key, since HCG results can be misunderstood. Here are the 10 things that matter most.

1. HCG is the hormone that signals early pregnancy

HCG is produced by cells that later form the placenta, after an embryo implants into the lining of the uterus. This is why it’s called the pregnancy hormone.

A home pregnancy test checks for HCG in urine. A blood test measures it more precisely and can detect lower levels earlier. For fertility patients, blood testing is routine in early monitoring because it gives more information (an actual score) than a home test.

Early HCG testing has its limits. The report (if you had the blood test) only shows one score, which can be hard to understand after a fertility treatment cycle. Is it too high or too low? Are you technically pregnant or not? What should you do next? These are common questions. 

2. One HCG result matters less than the pattern over 48 hours

This is the big one. Doctors want consecutive HCG tests, not a single result.

In early pregnancy, HCG levels often rise quickly. A common rule of thumb is that the score should roughly double every 48 to 72 hours. But every pregnancy is different. Not every healthy pregnancy doubles neatly. Many viable pregnancies see HCG levels rise faster or slower than average. 

Fertility patients tend to worry more about slower speeds. Very generally, low initial HCG levels after IVF treatment may negatively impact live birth rates. But the question for patients is: how low is too low? This is why doctors focus on the overall direction and pace of HCG, not myths about one perfect number.

If your clinic asks for repeat blood tests, 48 hours or so apart, it’s doing its job. We need to see the pattern. It’s the only way, before a scan, to tell if your pregnancy may be developing as it should be.

3. A low initial HCG score doesn’t mean bad news

This is where many fertility patients panic unnecessarily.

A “low” first HCG blood score, or a faint line on a home test, can happen for a very simple reason: you tested early. If implantation happened later than expected, or if your blood test was done at the earliest possible point, the number may be small. Patients should only really test after the two week wait. Many test earlier. This can bring its fair share of anxiety and confusion when the score is revealed.     

What matters is what happens next. A pregnancy that starts with a modest HCG level can still progress normally if the hormone is rising appropriately over time.

That’s why clinics don’t draw major conclusions after one early result. The first HCG score is data, not certainty.

4. A high HCG doesn’t automatically mean twins

This is one of the most common questions after IVF: do high initial HCG scores mean twins? It’s impossible to be sure. 

Multiple pregnancies can be associated with higher HCG levels, but this is far from conclusive. Some single pregnancies have high early HCG levels. And some twin pregnancies don’t show dramatically higher levels at first. There are too many variables. Only an early scan, around week 6 of your pregnancy, can tell you what you need to know.    

Very high initial numbers can also worry patients, often unnecessarily. In rare cases, unusually high HCG can be associated with conditions such as a molar pregnancy, but HCG alone can’t tell you that. A proper medical assessment is needed.

In short, high HCG levels after fertility treatment don’t, in most cases, provide certainty about how many embryos implanted or the viability of the pregnancy. A scan will. And a doctor’s interpretation.

5. Ultrasound is definitive – HCG isn’t

There comes a point, a fortnight or so after your post-embryo-transfer test, when scans matter more than numbers.

Repeat HCG tests are helpful, especially when an ectopic pregnancy is a concern. (The chance of having one after IVF is higher than natural conception.) But ultrasound scans are central to checking the safety and viability of your pregnancy. Transvaginal ones are best. It’s what you see that counts – not HCG numbers.

For fertility patients, that means booking a viability scan at the right point. Request images and a written report. It’s surprising how often sonographers exclude one or the other.    

6. IVF trigger shots can cause false positives

If you’ve had IVF using your own eggs, be careful with early HCG testing after a trigger shot containing HCG (like Ovitrelle). 

Some trigger injections used before egg retrieval contain HCG, and that hormone can stay in your body long enough to make an early pregnancy test look positive even when there’s no pregnancy yet. Clinics routinely warn patients about this. The trigger shot hormone can stay in your bloodstream for longer than you think.

This is one reason why early home testing after IVF can be emotionally difficult. You may be seeing the effects of medication, not implantation, and that’s hard to process or accept. 

If your clinic gives you a recommended test date, there’s a good reason for it. Following that timing usually gives a much clearer answer. Testing a little earlier when you used a non-HCG trigger shot (like Decapeptyl 0.1), or no trigger at all (for example, after a frozen embryo transfer) is more acceptable.   

7. Slow-rising HCG needs monitoring – but it’s not a diagnosis by itself

A slower-than-expected rise in HCG levels can be a sign that the pregnancy may not be developing normally. It might indicate a miscarriage has happened or may happen. Or be a sign of an ectopic pregnancy. But sluggish scores aren’t a diagnosis on their own.

Some ectopic pregnancies don’t follow a textbook pattern in terms of HCG acceleration – or lack of it. Many viable pregnancies see slow HCG rises. That’s why input from your doctor or clinic is invaluable. If your numbers aren’t rising normally, you may be asked to repeat the HCG blood test once or twice more – or bring your scan date forward.

8. HCG can flag an ectopic, but symptoms matter too

We’re focussing on ectopic pregnancies in this post for a reason. Fertility patients need to be careful. An ectopic pregnancy grows outside the uterus, most commonly in a fallopian tube. It can become serious quickly and needs proper medical supervision.

Seek urgent medical advice if you have a positive test and symptoms such as:

  • one-sided pelvic pain
  • shoulder tip pain
  • dizziness or faintness
  • vaginal bleeding
  • worsening abdominal pain
  • a feeling that “something isn’t right”

9. Online HCG calculators are only useful if used cautiously

Let’s be fair: not all HCG calculators are worthless. But many are oversimplified, poorly built, or inaccurate.

A calculator can show the percentage rise between two tests. That can be useful. What it can’t do is diagnose viability, rule out ectopic pregnancy, or replace clinical judgement. It doesn’t know your exact fertility history, scan findings, symptoms, medication, embryo age, or trigger-shot timing.

10. There’s no single “normal” HCG number

This is the golden rule, and it’s the key one worth remembering.

There’s no one “normal” HCG level that guarantees a healthy pregnancy. There’s also no “low” number that automatically means failure. Or a “high” one that signals trouble. Early pregnancy HCG values vary enormously between individuals and pregnancies. 

What your clinic or doctor wants to know is:

  • was the test taken at the right time?
  • is the HCG rising in a reassuring way?
  • are there any concerning symptoms?
  • what does the scan show?

That’s why, as discussed above, HCG monitoring usually includes two or more blood tests followed by an early ultrasound scan.

For fertility patients, the blurry nature of HCG scores is a challenge. You want certainty. HCG scores rarely provide it. What they do give is direction – a reasonable steer towards the next stage of the process. 

Final takeaway

If you’ve had a positive result after fertility treatment, try to focus on trends, not a single number. Rising HCG is encouraging. Falling or plateauing HCG needs review. Slow-rising HCG needs proper follow-up. And once the pregnancy is far enough along, ultrasound becomes much more informative than blood results.

They can be useful for calculating the rate of rise between two tests, but they can’t diagnose viability or rule out problems. They should be used as a rough guide only and never as a substitute for medical advice.

Get urgent medical advice if you have a positive pregnancy test and develop one-sided pain, heavy bleeding, shoulder tip pain, dizziness, fainting or abdominal pain. Those can be warning signs that need prompt assessment.

The clinic said this is perfect – no need to retest and wait for my scan. However looking this up on line this seems really high compared to other people and I am terrified there are more than 2 in there as there are 2 sets of boy/girl twins on my husbands side conceived naturally nieces/nephews and one of his brothers/sisters are also twins (it didn’t skip a generation!). Can you advise?

Did hcg quantitive test and came back at 374. Started to see bit of blood and some tissue. I went to the dr again yesterday about the bleeding which is not severe but there. She looked at my cervix and saw the mouth of womb is still closed but saw some blood still. I am not bleeding much like not even a panty liner full a day. She said it might be a threatening miscarriage. I went for another blood test today and results for hcg came back at 2000. Should i be concerned??

    1. I wouldn’t say spotting is always bad. I was ten weeks when I started spotting and was super concerned. But my levels kept increasing and the pregnancy went great. I didn’t bleed after that again.

    1. Did you get a reply ? I had hcg test 7 days after transfer level 7. Progesterone super low. Blood again 3 days after coming soon. Doctor said not to worry since it’s still early.

My husband advised me to do another test after 48 hours maybe it will pick it up but i just lost hope at all. Its so emotionally draining.

  55. Be encouraged! Stay positive, support each other and just seek information. My wife and I are on this IVF journey that actually started with a myomectomy last October. We started IVF in February and after egg retrieval and a bout with OHSS. So our transfer was on June 5th. On June 13th we had our Beta test and our results were “21”. My wife was in full panic mode. The nurse at our IVF clinic was less than encouraging and we lost optimism. I did research and found that no one is sure when the embryo implants so low Beta reading aren’t uncommon so, that first result can be low based on how many days in that 3-4 widow are the transfer did the embryo implant. Go figure why no one tells you that at the IVF clinic? Two days later we have the 2nd blood draw and our numbers were at 59. Three days later we were at 325. Then three day after that we hit 10,050. So don’t be dismayed no matter what these numbers are. My research revealed that healthy babies can be successfully delivered from a high range 25,700 – 288,000. So don’t let the numbers consume you they are guidelines with astounding variances. I wish you all positive results on your journey!

      I was devastated with negative thoughts.

    1. This is my same concerns because this is the first time I did FET. I actually did my FET 10/4/18, did my blood test today to see what my HCG levels were the nurse said I was at a 5. Doesn’t mean all is lost but in most cases theses kinds of pregnancy normally end up in a miscarriage.. I go back to the doctors Thursday to see if my HCG levels has increased. I’ve been having a very difficult time with the daily progesterone injections to the point where I can’t move around in the bed and I get stiff after the standing for long periods of time….. I’m

    4. We did 2 retrieval cycles and only got 3 genetically good embryos. We transferrred 1 two years ago and it failed. Then this year we did an ERA and it moved my transfer date 2 days later than it was the first time and I just got a day 5 genetically sound ET on 10/25/18 and they drew my HCG yesterday and it was 52!!! So there is hope! Hang in there!!!

  95. I had a fresh 3 day transfer. My first beta was only 6.05 on June 8th. It went up to 11.5 on June 10th, and up to 21.7 on June 12th. I go back in two days. Is there any hope for me? Doctors tell me not to get my hopes up, and are already planning another cycle.

Hello so I did my second IVF Cycle our first cycle no eggs fertilized so this was my second go and we got 13 eggs 10 fertilized and we did a fresh transfer and put In 2 day 5 I have my first blood work 6 days after transfer and by accident they did a HCG5 which wasn’t supposed to be done until Monday June 14th 2021 the results came back as a low positive the nurse said it was a 25 I am guess this is good ? But won’t really j ow until tomorrow has anyone had this happen before bloodworm to early and get a low positive?

  98. Had my 5dfet on 4 August 2021. 9dp5dfet my beta was 121. 12dp5dfet it was 330. We transferred 1 blastocyst. Wondering if it could be multiples?

    1. I had my 5dfet on August 13th, I’m 11 dp5dfet, and my HCG levels are 604. We did a single embryo transfer, he was a 5BB. We also had assisted hatching. Hoping it isn’t twins.

    2. It is a good number. Stay strong and happy.I am looking for hope and miracle mine is only 4.this is my 3rd ivf baby yet.But you stay happy.I am going to repeat it tomorrow on August 31 which is my 14dpo.just with crossed fingers. Me and my husband could not sleep.But just pray…..

Went through IVF treatment had three embryos transferred on October 30 and waited the 12 days and had a pregnancy test bloodwork done it showed positive the number was 41 had a repeat test three days later and it went down to 38 fertility specialist wants to do another test this upcoming Monday I’m just freaking out that maybe I am losing the pregnancy is it common for it to go down and possibly jump back up or because they put three embryos in maybe one or two didn’t take and lowered the hCG levels I am currently freaking out I am so emotional and upset

  100. Hi did my 2nd Ivf transfer on the 9 nov 2021 went for beta test on 20 November my doc just called to say the test is negative but I have to go and do second test I am so worried bruised and so down .

  101. Did a 5dfet on 23nov and our first bhcg came back positive on the 3rd dec -115, on the 6th it went up to 344 , the 8th 452 however ive started bleeding for the past 2 days and today 10th dec hcg dropped to 432.

    Is that a miscarriage?

  102. We transferred two embryos (both 4AB) on Jan 31 2022. The HCG levels were 232 IU/L and 470 IU/L on day 9 and day 11 post FET, respectively. II think the HCL level is high. Does it mean it might be twins? Some sources were saying it could be due to down syndrome! Is this true?

  103. Hi I took a pregnancy test about a month and 3 weeks after my period. It came back positive got loads done it was at 50 went for a scan the next day and nothing was detected following day had another test came back positive did bloods again on the same day again hcg came back at 300 I have another scan in 4 days because my first hcg was low as I was expecting to be around 6 weeks anyway long story short the lady who did my scan said I am possibly really early 1-2 weeks my doctor seemed to think so aswell. So being around 1-2 weeks what does the second hcg levels suggest ? I’m kind of confused oh I forgot to mention that 5 days ago I was a bit wet and had a light small amount of blood and for the next 4 days I did not bleed again until today which was a light pinky colour I will see doctor again tmrw to get another blood test to see what my hcg levels are at but yes any ideas I have had four children and this has never happened before p.s I am unvaxd.

    I went through my first FET (2, top quality 4AA, not sure 3-day or 5-day) on Jun-10-22 and after the 2ww, had the blood test on Jun-24-22. Blood was drawn and my doctor also scanned (transvaginal) and said that I’ve conceived. One looked prominent and the other one she said may come up later, looked like twins. That night, got my bhcg and it was 595. I next have to visit doctor for a scan on July-15-22. I am anxious to know if everything is alright and fine and normal as my next appointment is only after 2 more weeks.
    Please suggest.

  105. Hello everyone we did our embryo transfer dec 14th and our first blood test 23rd which came back at a 70. Today we did our second and it was 94 they nurse said they would of likes it to double! Should we be concerned? We go back in 48 hours to check again but cant help but worry worry worry!

  106. I got a positive pregnancy test and then went to the hospital to get it confirmed but I was also cramping. They did a blood test and stated my HCG was at 4.07. They stated I was not pregnant. They did not do anything about the cramping or were even concerned about it. I am still cramping today and started spotting am I having an early miscarriage?

  107. My LMP was 6 October… I got my IUI done on 17 October….my beta HCG was 31 on 4 November, 76 on 8 November and 140 on 14 November…will it be counted from 6 October or 17 October…. As the count is less if it is from 6 October but normal if from 17 October
